GREAT LAKES MANNING COMMUNITY OPTIONS PROJECTS-SENIOR CLIENT SERVICES OFFICER
PERMANENT PART-TIME (32 HOURS PER WEEK) $835.49 per week based on 32hours/wk
Great Lakes Manning Community Options Projects is auspiced by Great Lakes Council, functioning as part of the Community Services section. The services is funded by the Department of Ageing, Disability and Home Care through the Home and Community Care (HACC) program.
Great Lakes Manning Community Options Projects provides case management and brokerage support to approximately 118 HACC clients with complex needs who reside in the Great Lakes, Greater Taree and Gloucester local government areas. Staff provide case management and co-ordinate services and support to meet the needs of clients and their carers, and assist them to continue living at home and accessing the community.
Your primary role as the Senior Client Services Officer will be the provision of case management services to clients, including assessments, monitoring and support. You will work closely with the Team Leader, staff members, clients and carers linked with the program. You will be involved in service coordination, assessments and monitoring of clients and their individual needs. A major part of the position will be providing regular advice and support to up to 7 Client Service Officers.
Qualified in Community Services or a similar field at the TAFE Certificate level, your demonstrated competency in care coordination, service delivery and client advocacy will be highly regarded. As will your extensive experience in case management for frail older people, people with a disability and their carers, or other designated special needs groups with multiple complex needs. Excellent skills in communication and documentation will be highly regarded. Proven knowledge of Microsoft Office and specialised software is also required.
You are able to demonstrate the ability to monitor and perform within the constraints of a budget, as well as having a proven track record in negotiation and conflict resolution and a current Class C Driver's Licence.
Ideally, you have a knowledge of the Home and Community Care Program and Services in the local area.
Ongoing employment in this position is subject to the continuation of Government funding.Preferred candidates will be required to undertake a medical assessment and a police check as part of Council'sRecruitment Process.
