Great Lakes Manning Community Options

You may be thinking of moving into a nursing home or hostel simply because you can no longer cope with everyday tasks. You may already be receiving assistance but find it difficult to manage. Consider Great Lakes Manning Community Options Projects.

A CHOICE FOR STAYING AT HOME

What is Community Options?

Community Options is a client focused service aiming to provide flexible responses to your needs. This is achieved through case management and coordination of services. Community Options provides support, assistance, information and advocacy to help you to stay living at home.

How can this service help you?

Older people who are frail, and people of all ages with a disability who have complex needs may be eligible if they are:

  • Having difficulty coping at home and need services additional to those which are already available, or need their services co-ordinated, or
  • Thinking about going into a nursing home or hostel.

Community Options Funding

Community Options is jointly funded by the State and Federal Governments and is part of the Home and Community Care (HACC) program. Our service provides case management and brokerage to clients with complex needs across three Local Government Areas - Great Lakes, Greater Taree and Gloucester.

The service also has funding for three Acquired Brain Injury (ABI) clients across the three Local Government Areas. These clients also receive case management and brokerage.

We have further funding for one Service Access System (SAS) client. This client has very high needs and the funding is directly allocated to this person.

We Can Help!

We can help by:

  • Liaising with other community care services and supplying extra services to meet your needs.
  • Giving support and assistance to the person who cares for you.
  • Keeping in regular contact with you and monitoring your needs.
  • Supplying equipment to help you with everyday tasks.
  • Advocating on your behalf with service providers.

Who can refer you to Community Options?

Anyone can refer you to the service including relatives or other carers, your doctor, health workers, specialist assessment services or you can refer yourself.

Will you have to pay anything?

Your individual needs and appropriate services will be discussed and a contribution to the service will be negotiated with you at a home visit.

Can everyone be helped?

Some people may be so frail or disabled that they may require intensive and/or highly specialised care on an ongoing basis. This may be beyond the scope of this service.

However, if you have needs that are not being met because you are not getting:

  • Any assistance.
  • Enough assistance,
  • Assistance at the right times, or
  • The right assistance
    we may be able to help.

Great Lakes Equipment Pool

The Great Lakes Equipment Pool was established to enable people in the community to access affordable equipment for short-term periods, to assist them in their own home and in the community.

An example of equipment available includes commodes, shower chairs and stools, walkers on wheels, hi-lo chairs, over toilet aids and wheelchairs.

If you require equipment to assist you after a hospital visit, or if you would like to "try before you buy", the equipment is available to the Great Lakes community for a period of two months.

A contribution is payable when picking up the equipment from our office at the Community Centre, Breese Parade, Forster. For further information contact our office.

Objective:

To assist clients to remain independent in their home and community thus preventing inappropriate institutionalisation in the Local Government Areas of the Great Lakes, Greater Taree and Gloucester.

Mission Statement:

Great Lakes Manning Community Options Projects support frail, older people and people with a disability who have complex needs, and their carers, by assisting them to maximise their quality of life at home and in the community.
